Case Management

Subtitle

Mental Health Case Management offers coordination of services for children with severe emotional disturbances, enabling them to remain at home, and for adults with severe mental health issues, assist them in remaining in their community.  Services may include mental health, social, educational, health, vocational, recreational, volunteer, advocacy, transportation and legal services.

Hours

Office hours: Monday–Friday, 8:30 a.m.– 4:30 p.m.

Participants

Children with severe emotional disturbances (SED) and adults with Severe and Persistent Mental Illness (SPMI) who live in Hennepin (children only), Stearns, Benton, Mille Lacs, Wright, Isanti, and Chisago Counties counties.

Fees

Funded through Hennepin County, Medical Assistance, or Prepaid Medical Assistance Providers (PMAP)

How we help



Each participant’s family is assigned a case manager who assists them in determining what services are needed, helps the family obtain the services, assesses new needs, and coordinates with all service providers who work with the family to keep the services working effectively.
