Client information

Treatment Planning

All clients receiving services at Vona Center for Mental Health will be involved in the treatment planning process. If client is a minor, their legal guardian will also participate in the treatment planning process. The treatment plan identifies the agreed upon goals, objectives, and interventions.

Services for Minors and Parental Consent

Parents or the child’s legal guardians are required to give consent for treatment of a minor child. In cases where parents are separated or divorced but share legal guardianship, both parents are required to give written consent for treatment.

Cancellations

If it is necessary to cancel an appointment, 24 hour notice is expected. If an individual fails to keep an appointment and/ or late cancels 3 appointments in a 12 month period that individual may be transferred to our Same Day Scheduling.
